Your first session at Pura Vida Family Therapy is designed to lay the foundation for a supportive, ethical, and effective therapeutic relationship. During this time, we will review key documents and procedures to ensure transparency and mutual understanding. Below is an overview of what you can expect:

1. Informed Consent for Treatment

Therapy is a collaborative process aimed at promoting personal growth, emotional well-being, and mental health. At your first appointment, you will review and sign an informed consent document, which outlines your rights, responsibilities, and what to expect from the therapeutic experience.

Therapeutic Methods:
Our practice utilizes a blend of evidence-based modalities tailored to your individual needs. These may include Cognitive Behavioral Therapy (CBT), Trauma-Informed Therapy, the Gottman Method (Levels 1–3), Solution-Focused Therapy, and Person-Centered approaches. If desired, faith-based discussions can be respectfully incorporated in accordance with your preferences and ethical standards.

Client Rights and Responsibilities:
You have the right to:

  • Receive respectful, ethical care

  • Ask questions about your treatment

  • Discontinue therapy at any time

You are responsible for participating actively, being honest, and engaging consistently in the therapeutic process.

Session Length and Frequency:
Sessions typically last 53-60 minutes. Frequency is customized based on your therapeutic goals and availability.

2. Confidentiality and Its Legal Limits

Confidentiality is a cornerstone of therapy. However, there are specific exceptions mandated by law:

  • Mandated Reporting: Suspected abuse or neglect of minors, elders, or vulnerable individuals must be reported.

  • Threats of Harm: If there is a risk of harm to self or others, appropriate interventions may be required.

  • Court Orders: In the event of a subpoena, certain records may need to be disclosed.

3. HIPAA Notice of Privacy Practices

Your privacy is protected under the Health Insurance Portability and Accountability Act (HIPAA).

  • All records are securely stored and managed.

  • You may request access to your health information, with the exception of some clinical notes.

  • Information will only be shared with third parties (e.g., legal representatives, insurers) with your written consent, unless otherwise required by law.

4. Fee Agreement and Financial Policies

  • Session Rates: $100 for individual therapy and $150 for couples counseling (per 53-minute session).  $50 for group sessions (unless otherwise noted).

  • Payment: Accepted forms include cash, credit card, or electronic payment. Payment is due before the start of each session.

  • Insurance: I am currently [accepting/not accepting] insurance. Superbills can be provided for clients wishing to seek out-of-network reimbursement.

  • Cancellations: Please cancel at least 24 hours in advance to avoid a cancellation fee (50% of the standard session rate). Missed sessions must be paid before your next appointment.

  • Sliding Scale: Reduced fees may be available based on financial circumstances. Please inquire if needed.

5. Telehealth Services and Consent

Online sessions are available through a secure, HIPAA-compliant platform.

  • Please ensure you are in a quiet, private space during sessions.  Do not attempt to complete video sessions while driving.

  • In case of emergency during a virtual session, you are responsible for contacting 911 or the nearest crisis center.

6. Court-Ordered Counseling Guidelines

For clients participating under a legal mandate:

  • Compliance: Active attendance and participation are necessary to meet court requirements.

  • Reporting: Progress summaries may be submitted to the referring court or agency.

  • Missed Sessions: Absences may be reported as noncompliance.  If you must miss a group session, please reschedule an individual session & pay the individual rate before the next scheduled group session.

7. Client Records and Access

  • Therapy records are maintained for at least seven years following the conclusion of services.

  • Clients may request copies of their records by submitting a written request.

  • Third-party access requires a signed authorization form.

8. Crisis and Emergency Services

Pura Vida Family Therapy is not a 24/7 crisis service provider. In the event of an emergency:

  • Call 911 or the Suicide and Crisis Lifeline at 988

9. Professional Boundaries and Social Media

Maintaining healthy professional boundaries is essential, especially in a close-knit community.

  • Encounters outside of sessions will not be acknowledged to protect your privacy unless you initiate contact.

  • Social media interactions (e.g., follows, messages to my personal account) are not permitted.

  • Please reserve emails and texts for scheduling and administrative matters only.

Everyone goes through challenging situations in life, and while you may have successfully navigated other difficulties, there’s nothing wrong with seeking additional support when needed. In fact, therapy is for people who possess enough self-awareness to recognize when they could benefit from a helping hand. That insight is something to be admired. By exploring therapy, you're taking responsibility for your well-being and committing to meaningful change. Therapy provides long-term support and tools that can help you manage triggers, shift unhealthy patterns, and move forward through life’s challenges with greater confidence and clarity.

Depending on your needs, therapy may be short-term—focused on a specific issue—or longer-term for deeper personal growth and lasting change. Most clients benefit from meeting on a regular schedule, often weekly, to build momentum and maintain consistency.

Active Participation is Key

Therapy is most effective when you’re actively engaged in the process. Your therapist may suggest activities outside of sessions to support your growth, such as journaling, reading, mindfulness practices, or taking practical steps toward your goals. These assignments are meant to reinforce the insights and strategies discussed in therapy.

Ultimately, therapy is a collaborative process. It works best when you’re open to new perspectives, committed to personal development, and willing to take responsibility for the changes you want to make in your life.

While medication can be an important tool in managing symptoms of mental health conditions, it is not typically a long-term solution on its own. Therapy addresses the underlying causes of emotional and psychological distress by exploring thought patterns, behaviors, and past experiences that may be contributing to current challenges.

Psychotherapy empowers individuals to make meaningful changes, develop healthy coping strategies, and build emotional resilience. Rather than simply alleviating symptoms, therapy works to uncover and resolve the root causes of distress—leading to more lasting improvements in overall well-being.

An Integrative Approach

In some situations, the most effective path to healing may involve a combination of both medication and psychotherapy. Medication may help stabilize acute symptoms, allowing you to engage more fully in the therapeutic process. If medication is recommended, I will gladly provide referrals to trusted psychiatrists or medical professionals who can evaluate and prescribe as needed.

Ultimately, the decision to include medication in your care plan is personal and best made in collaboration with your medical provider. At Pura Vida Family Therapy, we believe in a whole-person approach to mental health—tailored to your unique needs, values, and goals.

At this time, Pura Vida Family Therapy does not directly accept insurance. However, many clients are still able to receive partial reimbursement through their insurance provider by submitting a 'Superbill.'

What Is a Superbill?

A Superbill is a detailed receipt provided after your session that includes essential information your insurance company needs to process an out-of-network claim. This document typically includes your name, diagnosis code, treatment code, session date, and the total fee paid. Submitting a Superbill allows you to seek reimbursement according to your insurance plan's out-of-network benefits.

It’s important to contact your insurance provider directly to ask about your out-of-network mental health benefits. Key questions to ask include:

  • Do I have out-of-network benefits for mental health counseling?
  • Is therapy with a Licensed Marriage and Family Therapist (LMFT or equivalent) covered?
  • What percentage of the session cost will be reimbursed?
  • Do I have a deductible that must be met before reimbursement begins?
  • Where should I send my Superbill for processing?

Payment Options

All session fees are due at the time of service. Accepted forms of payment include cash, credit/debit cards, and electronic payments. Rates are detailed above in #4. If you would like to request a Superbill for reimbursement, please let me know and I will provide one after each eligible session.

Why I Do Not Bill Insurance Directly

There are several reasons for not billing insurance directly, including maintaining greater confidentiality and allowing you to have more control over your treatment. Working outside of insurance also enables us to tailor your therapy without the limitations and restrictions that some insurance providers may impose, such as session limits or diagnosis requirements.
